Our vision is of a society in which disadvantaged people can lead full and independent lives fully participating in society.
Our mission is to promote the equality, dignity and independence of disadvantaged people.

The overall aim of the Carers Health and Wellbeing Service is to improve the quality of life for Carers in Wirral, supporting them to sustain their caring role and enhancing their ability to enjoy a life outside of their caring role.

Most support services are provided by independent, voluntary or third sector providers who receive funding from the council or the Integrated Care Boards.
Some of these free services can be accessed directly without the need for a social services assessment.  However, access to some other services requires an assessment to see if you are eligible for support.
We aim to help with your caring responsibilities so you can maintain some independence and balance in your own life.
This support could be:
Information and advice about support that is available
Referral to Carer support services in the community
A personal budget or grant to help meet  some of your own needs

Wirral University Teaching Hospital, recognises and appreciates the vital role Carers play in the life of all of our patients and we have joined the Regional Carers Passport scheme, We want to promote a culture that recognises Carers and their needs within our hospital.

The Carer Passport supports staff to honour our carers pledge by acknowledging the patient has a carer in place and values their input. Whilst you are providing carer support at the Trust, we will ask you what support you as a carer may need and will strive to support you or help you find relevant support.

The passport is a free ID issued by WUTH, once a carer has requested one with one of the clinical team of the patient. All carers will have to complete a Carer Passport Partnership Agreement, that they can discuss with the care team and then they will be issued the ID card.

We explain how things work and navigate the systems and processes on their behalf, until they’re ready to do it for themselves.
In short, we open doors to essential services that are available to everyone.

Social Prescribing Logo_edited_edited_ed

Social prescribing Link Workers are present in surgeries across the Wirral to help patients identify and link with sources of support within the community. These non-medical referral options can complement existing medical treatments to improve health and wellbeing.

Social Prescribing Link Workers are not medical professionals but have been specially trained to work alongside GPs and other clinical roles to ensure people get the care and support they need.
